Who this is for and when to use it

This template is for reliability engineers, maintenance engineers and planners developing or improving PM programs. Reach for it when a piece of equipment keeps failing and you need a structured way to work out why and what to do about it, when you're rebuilding an old or OEM-based PM program around what actually fails, or when you're specifying maintenance for new equipment during the design phase.

Why it matters

A failure modes and effects analysis is usually one of the first steps in improving the reliability of a system or piece of equipment: break the equipment down, work out how it could fail, why, and with what consequence, then assign the actions that prevent it.

Most FMEA templates miss the characteristics that determine whether a PM task will actually work. Ours makes you capture them for every failure mode: is it a hidden failure? Is it age-related? Does it have a consistent P-F interval? Knowing this is the difference between PM tasks that prevent failures and PM tasks that fill a schedule.

Step by step

How to use it

  • Plan and scope the analysis. Pick the equipment causing you the most pain, not everything at once.
  • Break the equipment down and identify failure modes, effects and causes. The process follows IEC 60812, simplified for practical use.
  • Capture the failure mode characteristics the template asks for. They determine which PM tasks will work.
  • Assign preventive or corrective actions and feed them into your PM program. Our article on FMEA walks through each step.
  • Remember: not every failure mode can be managed by maintenance. Some need a physical modification. The analysis will tell you which.
